T-KDZ9’s 20 capability instances all arrived with need_human_review: true (a v2-migration leftover). AC-5’s bijection (uncertain PR-table rows == need_human_review: true files) required flipping the 15 confident gradings to false, repurposing the flag from ‘ungraded’ to ‘close-call grade’. This curation wasn’t in the task’s Files-to-touch. A task that grades into a confidence field should state up front how pre-existing flag values are handled, so the scope is explicit.

Closed 2026-08-16: superseded by D-VSLI-distributed-work-runner-architecture. Ledger phase C1 makes spec depth the routed process’s contract rather than a universal template, so a cross-cutting rule about what a task spec must state up front has no universal template to sit in.
